
United States health officials have declared the country's largest recorded multistate cyclosporiasis outbreak over. The outbreak was linked to contaminated iceberg lettuce supplied from central Mexico and led to a major recall, thousands of reported illnesses, hundreds of hospitalisations and two deaths.
Reuters reported 12,883 cases across 21 states and 570 hospitalisations. The recalled lettuce is no longer expected to be available in stores or restaurants, which is why authorities can close the active outbreak investigation even while broader work on food-safety controls continues.
“Outbreak over” does not mean the parasite has disappeared from the United States or that every stomach illness is now unrelated. It means new cases connected to this particular source have fallen and the contaminated product is no longer circulating.
What is cyclosporiasis?
Cyclosporiasis is an intestinal illness caused by the microscopic parasite Cyclospora cayetanensis. People usually become infected after consuming food or water contaminated with the parasite.
Fresh produce has been involved in previous outbreaks because it is often eaten raw and may pass through several stages of growing, washing, packing and distribution. Unlike an infection that easily spreads directly between people, Cyclospora generally needs time in the environment before becoming infectious.
That biological detail changes public-health tracing. Investigators focus heavily on shared food exposures, suppliers and distribution records rather than assuming a sick person directly infected close contacts.
What symptoms should people watch for?
The most common symptom is frequent watery diarrhoea. Other reported symptoms can include loss of appetite, weight loss, stomach cramps, bloating, nausea, fatigue and fever.
Symptoms may begin roughly a week after exposure, although the timing can vary. Without treatment, illness can last for weeks and may appear to improve before returning.
Anyone with persistent diarrhoea, dehydration, severe pain or worsening weakness should contact a healthcare professional. Older adults, young children and people with other health problems may be more vulnerable to complications from fluid loss.

Is recalled lettuce still on shelves?
Officials said the affected product is no longer available in stores or restaurants. Fresh lettuce has a limited shelf life, so recalled lots from the outbreak period should have moved out of commerce.
Consumers should still check any recall notice that applies to a product they stored unusually long or froze as part of a prepared meal. The brand, lot code and purchase date are more reliable than appearance because contaminated produce may look and smell normal.
New iceberg lettuce on sale now is not automatically part of the outbreak. Avoiding all lettuce indefinitely would not reflect the public-health conclusion.
Why did the outbreak become so large?
Fresh produce supply chains move high volumes quickly across state lines. A single source can reach restaurants, institutional kitchens and supermarkets before investigators recognise a common pattern.
Cyclospora also creates detection challenges. Symptoms resemble other gastrointestinal illnesses, not every patient seeks care, and specialised testing may be required. By the time cases are linked, the original meal may have been eaten weeks earlier.
Better genetic and epidemiological tracking can help connect patients and food sources faster. Experts have also called for sustained research funding because parasites receive less surveillance attention than some bacterial foodborne diseases.
What should restaurants and suppliers learn?
The response does not end with a recall. Growers and processors need to examine agricultural water, worker hygiene, sanitation, washing systems and the physical design of facilities.
Restaurants should preserve supplier records and act quickly when a recall is issued. A bag labelled simply as “lettuce” inside a kitchen is harder to trace than produce connected to a documented shipment and lot.
Supply-chain transparency matters because contamination can occur before food reaches the final business. The goal is not only to remove a product after illnesses begin but to identify the point where the parasite entered the chain.
What happens after an outbreak is closed?
The CDC and FDA can publish final case information, complete inspections and recommend preventive changes. Enforcement or import measures may follow if investigators identify violations or recurring risks.
Healthcare providers may continue to diagnose sporadic cases unrelated to the closed outbreak. Patients should tell a clinician about recent travel, foods eaten and whether others who shared a meal became ill.
For consumers, the practical steps remain familiar: follow recall notices, refrigerate produce, wash hands and preparation surfaces, and seek care for persistent symptoms. Washing fresh food can reduce some contamination but cannot guarantee removal of every microscopic parasite.
The official closure is reassuring because the identified risk has been removed from normal commerce. The unusually high case count, however, makes this outbreak an important test of how quickly the US food system can detect and contain contamination in widely distributed produce.
